For patients wishing for an appointment, please contact your primary care provider and ask to be referred.

Please fax referrals to 919-881-2026.

Please include:

• Clear reason for referral and name of referring provider on the first page
• Patient demographics
• Insurance information
• Copy of all relevant labs, imaging, and most recent chart notes
